Hello,

In order to re-activate your account we will need you to sign this agreement and attach a copy of your Drivers License and Credit Card to this billing ticket. By adding your name on the line below for 'Cardholder Name', you agree that the electronic submission of your name shall suffice as your signature and authorization for the charges incurred as a result of the chargeback performed by you.

By signing this document you are agreeing to pay the amount due on the account that resulted from the chargeback you performed with the credit card in addition to any fees incurred per our Terms of Service Agreement (http://www.lunarpages.com/legal/ - Section 1.5). You also agree that after making payment that you will not perform a chargeback on the new charge. These funds will be paid to Add2Net, Inc., Lunarpages Division.
