Hello,
Ok, here is the link.  The former client is still getting some data back,
and I know you have waited a long time, but we can have you up within 24
hours or so once you complete this:

Choose the option "Transfer My Existing Domain's Hosting Only. I Will Update
my Nameservers." and the rest is pretty straightforward.

Here is how our partitioning will be:

Large / partition on first drive
400MB /boot partition
18GB swap partition for 12GB RAM

2nd drive will be left unpartitioned in its entirety.

If you need any changes to this partitioning scheme, PLEASE PUT IT IN THE
COMMENTS AREA.  Re-installs are $60 if you need them for reasons beyond
something that we caused or failed hardware so we want to get it right the
first time.

Also, unless you go for the $15 / mo optional management, you will have to
setup/install items like OpenVZ, HyperVM, and WHMCS yourself.  The server
will be considered "self-maanged".  This includes you setting up a software
firewall.  Otherwise, let me know and I can modify it to a fully managed
server for an extra $15 / mo.
